(In hospital)Ventilator

I have two questions. The first question is,Which one is (A) She have to put on a ventilator or (B) She have to be put on a ventilator? When someone have to put on it, a nurse or a doctor put on it to a patient. So I think that (B) is better...I'm not sure.

The second question is, What is the opposite of " put on " in this case? take off?
Can I say " She took off a ventilator " ? Could you tell me about them.
